Member Forms

Winning 4-H Plan Request - We strive to make quality 4-H Programming accessible to all youth in Preble County. With this in mind we can make accommodations in judging events and other activities so that every child can have a successful experience. In order to make sure that we know what accommodations are necessary, I am making a form available for parents to complete. We can use this information to communicate to judges and volunteers as needed. It will also help us to formalize our process so that we do not miss anyone. If a 4-H member needs accommodations for a show or project judging, please complete this form and return it to the Preble County Extension Office by May 15th or as necessary.

Across County Line Membership Policy and Request Form

Horse Permission to Participate Form - The Horse Permission to Participate Form must be signed by all members taking horse projects. Submission deadline for completed Horse Permission to Participate Forms is May 15th.

Dog Permission To Participate Form - The Dog Permission to Participate Form must be signed by all members taking dog projects. Completed forms should be forwarded to your club's organizational advisor. Club submission deadline for Dog Permission to Participate Forms is April 1st at the Preble County Extension Office.

Preble County Dog Project Identification Form and Vaccination Certificate - The Preble County Dog Project Identification Form and Vaccination Certificate is to be completed for each dog participating in the Preble County 4-H Dog Program. Completed forms should be forwarded to your club's organizational advisor.

Preble County Food and Fashion Board Application - If you are interested in being considered for selection as a Food and Fashion Board Member please return the completed application to the Preble County Extension Office.

Preble County Food and Fashion Board Reference Form - Completed forms should be returned directly to the Preble County Extension Office by the person providing your reference.
